Hacia una medicina personalizada y sostenible

Abstract

El concepto de 'medicina personalizada' se encuentra recientemente en boca de todos en los ambientes biomédicos. Este cambio de paradigma en el modo de entender la sanidad se plantea como la capacidad realizar acciones “a medida” de cada paciente, siguiendo sus preferencias y características únicas durante todo el proceso asistencial, incluyendo prevención, diagnóstico, tratamiento y seguimiento posterior. Este cambio tiene importantes beneficios de salud y bienestar para el paciente: tratamientos más efectivos y precisos, diagnósticos más detallados y prevención ajustada a su biología, historia familiar y estilo de vida. Pero también tiene implicaciones para el sistema sanitario, que puede optimizar recursos con políticas mucho más dirigidas y efectivas o estrategias de prevención basadas en evidencias biológicas.

El pilar fundamental que permite llegar a este nivel de detalle y individualización es la información almacenada en el genoma de cada persona. Esta información, alrededor de los 600Gb, tiene múltiples aplicaciones clínicas durante todas las etapas de la vida de una persona, pero viene acompañada con algunos desafíos: ¿dónde la guardamos?¿cómo se la proporcionamos a los expertos sanitarios que deberán interpretarla?¿cómo analizamos cantidades de datos tan masivas?¿cómo garantizamos su protección?¿y la privacidad del paciente? y lo más importante ¿cómo podemos afrontar este nuevo paradigma de forma sostenible para toda la sociedad?

Genomcore, presenta una propuesta basada en un sistema transaccional en el que los datos genómicos se almacenan de forma segura con un sistema propio de encriptación, y aprovechando la escalabilidad y capacidades de computación que ofrece su tecnología propia. Con este modelo, expertos en genética y profesionales sanitarios pueden acceder a partes del genoma del paciente para que sean interpretadas y utilizadas en acciones de medicina personalizada (de forma asistencial o investigadora), mientras el paciente mantiene el control de cada acceso mediante un sistema de consentimiento informado electrónico basado en blockchain. Este proyecto esta actualmente en tratativas con el gobierno de Dubai para la creación del sistema nacional de salud más avanzado del mundo.

ACTIVAGE - Report on IoT devices

Abstract

In order to have a complete Architecture of the ACTIVAGE platform, the device domain constituted of smart sensor and actuator nodes, gateways, connectivity and its associated protocols must be also considered. Therefore, it is imperative to identify the different devices required to implement the uses cases considered in the different Deployment sites (DSes). A systematic analysis is performed on the device, gateway, cloud and applications domains constituting each of the nine DSes. This analysis is based on several registration forms in order to gather key elements of each domain. The original goal was to collect information concerning only the device domain. However during the first months of the project, this goal changed to also include the three other domains in order to get an overall, homogenized, and rich technical information view of each DS. The additional information includes, the DS topology, the applications foreseen, the security and privacy mechanism to be implemented in the overall system, the servers used and their locations, and other information required to facilitate, in Task 3.2, the Security and Privacy assessments. Furthermore, this document gives a summary and classification of the different devices used in each DS allowing the identification of communalities, potential synergies and knowledge sharing between DSs. For the cases where, no suitable solutions are available, new devices could be prototyped in order to support a given use case with the right device or the right security protection level. Finally and in order to have a more complete document, the following aspects are also included: a list of key concepts in the device and gateway domains are explicitly defined to be shared and used in the overall project, a short introduction on the evolution of the communication systems is given in order to understand key IoT concepts at IoT architecture level and in particular at the device and gateway domains, including some security ones, and a brief market analysis on the home automation and health care devices.
